My husband and I are in an interlocking IVA, this is our 5th year.
Today we have each received from our IVA company a letter asking us to contact the tax refund company to review the tax codes HMRC has given us to identify if we have been given a wrong code. The letter explains that 1 in 3 people over the last 4 years have been given a wrong code.
We are both civil servants and wouldn’t have thought are codes would be wrong.
Has anyone else ever received a similar letter and is it something we have to do?
Seems a bit odd - I'd speak to HMRC and check to make sure your codes are correct before doing anything else.

This is a new one on me. I, too, was a civil servant and, as you know, they have a dedicated tax department so mistakes are less common than in the regional tax offices ( but, I guess, they still happen).
Might I ask which Insolvency firm you are with ?
