You must speak directly to the IP and find out what they are doing to sort this out. Being told that the meeting was approved is a shocking mistake and your IP has a duty to resolve this.
Speak directly to the IP because with this rejection you could be made bankrupt if that clause has been inserted. Do not deal with anyone other than the IP as ultimately you pay their wages and they need to get this sorted out before the next adjournment date.

below is a letter I have just sent to Gemma, I hope its ok to post
Dear Gemma

I am a member of IVA.co.uk and have been given your name in the hope that you can assist me.

I recently made a full and final settlement offer that came from my Mum. The original meeting was on wednesday 28th March. From what I understand most of the creditors voted, apart from the Insolvency Exchange, that were voting on 9. They asked for the meeting to be adjourned until yesterday, 4th April.

Shortly after the meeting time of 3pm I was advised that the offer had been accepted, but I had a call from xxxxxxxxxxx this morning saying that errors had been made at your end and infact the offer was rejected. This news has caused me a considerable amount of stress and I cannot understand how such a massive error can be made when you are dealing with such important issues.

I am already on medication for High blood pressure and depression and this has now made the whole situation worse. I have emailed xxxxxxxx a letter from my Doctor this morning confirming these statements. I did not send before hand because I only managed to pick it up from the Doctors yesterday and after hearing that my offer from my Mum was accepted I did not feel there was any point in sending it.

Now Im not sure what is happening, again I am having to wait around for some support and answers as to why this error could have happened. I know its easy for Payplan to just say sorry but that does not really help me sort out the situation.I feel totally devasted and only hope that you can turn this around for me and organise a further meeting to bring this to a proper conclusion,
