Just received this form and the majority is filled in with figures provided last year but there's nothing down for school meals or phone and internet . There's a column for me to add these figures but wondering why they're not filled in . Any advice please ?

No idea, just fill them out with the amount and who to, I am sure they will be taken into consideration when they work out your payments. If you don't put these bills in then you may end up having to pay more to the IVA pot.

Did you put these figures down last year?

My guess would be that these particular categories tend to fluctuate more than most.
