Early this year our full and final offer was turned down, because 2 creditors (27%of total) thought it too early to review(30 months). They rather surprisingly thought that a new mortgage offer may be forthcoming, this would mean someone offering £178000 to me earning £28000 and my wife £4800 both in an IVA.
In 3 months our 4th year review is due, with us having to obtain the extra £53000 mortgage, appart from the obvious refusals, what else in terms of documentation in addition to the usual review information will be requested?

If you have the refusals it is likely that the IVA will be extended although this will depend on the terms of the IVA. Creditors may accept a full and final at this time as the mortgage market shows no sign of changing.
